Please fill out the following form and enclose it with the tester. This will help us expedite the requested service. If you still have the packing foam that your tester was sent to you in, please reuse it. If you do not, put the tester in a box that allows you to put at least 2 inches of packing material on all sides of the tester. Please send the tester to the following address:
